You're referring to not less than 2.5 different "rules" in the attachment.
The output for "rules" and the definition for different variables are both left implicit; also there are "recurrence" and "scoring scales" that don't appear to be related to the rest of the definition.
Can you post a spreadsheet that more clearly defines what explicit output you want?
The file you have posted is stamped "company confidential". It should not have been posted.

I didn't even notice that until you pointed it out
I have removed that file, it says it is confidential.
Also, Please upload a sample of your workbook, not a picture of your data. Pictures are pretty much impossible to edit, and no-one wants to re-type your data for you
Also, not all members can upload picture files (Company firewalls and stuff)
Your workbook should show a small desensitized example of the data you are working with and a manual mockup of the expected results.
