    convert raw data into structured column for repeating data

    Hello - I have some data with 2000+ rows that has repeating information. I would like to take this and put into columns that I can use for mail merge etc.

    On the attached sample file, everytime there are words "provider ID", there is name of facility, dentist, address and phone number following that. I would like to take those and put them in those columns. One of the biggest challenge is that address etc. are not always fixed in terms of number of rows and it could vary.

    Does anyone have any macro that I could modify or ideas that could get me started?

    The attached workbook has the macro below added. There is button on the worksheet to run the macro.

    The macro is designed for this workbook's layout. If you plan to use this macro in another workbook, some changes may need to be made to the macro.
