Populate complex data in listbox

Hi Dear

I have trying to populate a data in a listbox1

i have five columns with names and records A,B,C,D

Every name can have at least 1 record and maximum 4 records

I want to populate the data in a listbox.
I need to repeat the name in each record.

I have attached the file with the desired output.
