Customizing and getting a report from a database using a query form.

Could any one help me here?

I have been working on customizing a userform that acts more like a query form and can generate reports based on the options selected in the field. Please find attached a sample report. I would like to know if it is possible to make dynamic selections using dependent combo boxes and get a report through the user form on a seperate worksheet within the same workbook. PFA an excel sheet which gives you an overview of the data that I have which is quite extensive.

Would appreciate if someone could give me an outline in starting with this!

In order to attach your file edit your post, Go Advanced and under the textbox area (lower-screen) there is a "Attachments - Manage Attachments"
section. Try uploading your file there. As for

Thanks Bogdon! But the attach button does not seem to work. I am not sure if its a problem with the browser I am using.
