I think that you should give us more examples of your data. Can you have
Smith John A.
Smith III, John A.
Smith, J.

part way there..
=UPPER(RIGHT(A1,LEN(A1) -FIND(",",A1))&" " &LEFT(A1,FIND(",",A1)-1))
will have to think about the initial
Frob first, tweak later
=upper(A1 & " " & B1)
got the initial, lol
=REPLACE(UPPER(RIGHT(A1,LEN(A1) -FIND(",",A1))&" " &LEFT(A1,FIND(",",A1)-1)),FIND(".",UPPER(RIGHT(A1,LEN(A1) -FIND(",",A1))&" " &LEFT(A1,FIND(",",A1)-1)))-1,3,"")
BtW, this will do your example:
=UPPER(TRIM(LEFT(SUBSTITUTE(MID(A1,FIND(" ",A1)+1,255)," ",REPT(" ",500)),500))&" "&SUBSTITUTE(A1,MID(A1,FIND(" ",A1)-1,255),""))
