I gone through your file. Kindly confirm data extract from which sheet. I only found "Channel - Measure of Success" sheet. & another Channel Evaluation Sheet. Two sheet "hide".

As i thinking i have created sheet "Channel - Measure of Success". Plz look into all other columns. I using simple index match formula base with "OID". I suggest you instead of "OID" go with "Site ID", because of duplicate "OID" number, whereas site id is not duplicate (unique).
