Setting the cursor at desired spots for a form

Hi Everyone,

I created a form that I have secured but would like people to enter the requested data after each field. What I would like to accomplish is to have the user tab to each field entry. However, I haven't had any luck setting the cursor at the desired spots. Could someone show me how to setup a document so I can have the user tab to the desired fields and the cursor will be located in front of each field? I have enclosed my form I have created the protected and the unprotected version. Thank you for your help.

I deleted the fields (?) that you inserted and replaced them with plain text controls from the Developer Tab on the ribbon.

The request details field is set to accept carriage returns so that multiple lines can be entered.

I moved the signature line to the footer so that it will always be at the same location no matter how many detail lines are used.
