to make database to store the employees's appraisal report monthwise

I have 40 people working with me. I want to create a data base for creating and storing the monthly appraisal report of these employees. It should contain employee's staff no, name, designation, assigned area, month and year for wich the appraisal is made etc.

When I want to create an appraisal for a particula employee and for a particula month, by selecting his staff no or name the data base shoul open a form where I can make his appraisl. When I want to open and see an employee's report, I should be able to open it by clicking his staff no and month.
