    9 Box for Performance Matrix

Hello, I am looking to create a dynamic 9 box for Performance Ratings. We would like to have 1 tab for a 9 box using column K, 1 tab for a 3 box (A,B,C) using column I and another tab for a 3 box for (+/-) using column J. The attachment called 9 Box Source File Final is what I am looking to achieve. How do I create the dynamic chart off of the lookup tab?

Also, I think I have created my lookups corrects but not 100% sure. I am attaching a sample workbook (Test Data File) if anyone can help.
