3 types of information in a bar chart?

Hi! I've never posted here before.
I recently got an office job and had to learn excel. It's gone flawlessly for the most part, but today I got a task that doesn't make much sense to me.
I'm trying to figure out how I can make a bar chart (whether it is a stacked column or regular column) that will display 3 types of information.
I work in a factory, and I'm trying to figure up scrap over 3 months. So here is the information I would need, if anyone would help me figure this pickle out.

-January, February, and March are 3 months I need scrap from

-I need the top 3 reasons for scrap. (examples: nonfill, tears, breaks)

-and lastly I need the number of said scrap.

I'm guessing this is probably very simple, but my pea-brain no comprehendo.
    Thanks!

    To attach a Workbook
(please do not post pictures or links to worksheets)
    • Click Advanced (next to quick post),
    • Scroll down until you see "Manage Attachments",
    • Click that then select "add files" (top right corner).
    • Click "Select Files" find your file, click "open" click "upload"
    • Once the upload is completed the file name will appear below the input boxes in this window.
    • Click "Done" at bottom right to close the Attachment Manager.
    • Click "Submit Reply"

Here is the information I've compiled, and the failed chart I came up with. If you feel you can help, I'd love you.
And in regards to the last chart examples, thats EXACTLY what I need. But all of the months have different causes. That's the part I'm having trouble incorporating into it.

Reformat your data and then you can create a view that makes sense by using a pivot chart

Sounds like a simple data arrangement and a simple chart will give you what you want.

    Defect type in header row.
    Month in header column.
Values for defect type per month in intersection of rows and columns.
Insert chart type, may need to switch rows and columns.
