Generate visual matrix from formatted data

Hello everyone,

I'm trying to generate a visual matrix using a set of data formatted using an IF/AND function.

The table is designed to compare the engineering functions of a box with the risk of an operation contained within it, and display whether the box is under engineered, tolerable, broadly acceptable, or over engineered.

• Sheet "Data" contains the table displaying/listing all of the conditions of a box
• Sheet "Matrix Raw Data" contains the formatted data after the if function has been applied. The IF/AND function compares the risk of operations to the engineering capability score, and generates the operation name as the result if both conditions are met. For example, column B looks for boxes that meet both the criteria of failed engineering and low risk operations, column C looks for boxes that have failed engineering and medium risk operations, etc.
• Sheet "Matrix Formatted" is the template I have created for the raw data in "Matrix Raw Data". I need to copy all of the operation names that match a certain criteria into the correct cell. e.g. All operations that meet the criteria of "Failed/High" (Column D in the "Raw Data") sheet need copied into cell C6.

Is there a way to do this easily? I experimented with a concatenate formula but did not get anywhere with this. Ideally I need to keep all of the operation names on separate rows also.
