    Form data entry

    Good afternoon.

    I have attached a copy of my spreadsheet for you to look at.

    Figure 1 shows how my data is going to be displayed. I am trying to
    make a form that will allow me to enter data and have it push the
    existing data to the left as shown in Figure 2. This way, when the data is presented
    there is always 4 quarters worth of data showing.

    I am unable to build a form that does this for me. Any help would be greatly appreciated!

    This may be easier to accomplish with a custom userform as opposed to the automatically generated ones.

    We can help with that but will require a sample workbook as a starting point and more info on how you need it to work.

    Thank you for the response. I have attached a sample spread sheet for you (Test Form Entry). The Sheet labeled "HCAHPS Dashboard" is where I want the Quarterly data to populate automatically from a form or database. I want to be able to enter quarterly data and have it populate into the dashboard. every time I enter a new quarters data, I would like the previous to shift left leaving the new quarter and the previous 3 quarters visible on the dashboard. I have also reattached the sample from my initial message. Let me know if you have any other questions, and thank you for the help.
