Which bit of the above are you not able to achieve? Presumably you understand the basic concept of a spreadsheet and how to arrange your data (names down the left hand side, assessments across the top)?

A quick search via GOOGLE, using the following search terms ( excel example grade book ) provided thousands of free, complete examples ready to be downloaded.
Regarding the need for student photos / personal information .. consider searching for a HUMAN RESOURCES EMPLOYEE DATABASE example. Again, there are many
free download examples on the internet.
After finding what you are comfortable with (to be applied to your needs there) ... when you encounter an issue modifying the free downloads ... come back with your
questions. There are plenty of volunteers here willing to assist.
Thanks for the response. The aspect giving me difficulty is how to produce result sheet for individual student from the grade sheet. I have prepare the score sheet.
Let's see the score sheet, then. Names need to be desensitised (so change to Person 1, Person 2, etc.).
