Match and Index help with formula

Hi All
Am hoping someone can help me with this, my formula below isnt working and i cant work out why. I have entered it as an array.

=(INDEX(Data!$V$1:$V$1000,MATCH(1,(Data!$Q$1:$Q$1000=D2)*(Data!$U$1:$U$1000=$F$1))))

I have attached my sheet i have changed the details but essentially where the name matches pass I want to return the value in column V.
