Search under a date date range

Hi

I am looking for a formula that can search for the result under a date that fits certain search criteria.

The attachment has an example.

I am using the results to populate a graph eventually.

I used a macro to rearange the data, so they are easily put in a pivot table.

Push on the button on the output sheet and the date will update.

After that I made an graph of the data (as example, it also can be a line-graph).

Probably Vikas_Gautum put me on the right track.

I changed the macro, so there are also the values of the weekly in the table.

In the pivot table, you can also change to other jobs.

That looks like a nice solution from Vikas. However, he did not add in your requirement also to match against the term "Provider". I have added that into his formula.
