Got the solution..
Check the attachment..
Don't forget to click *
I used a macro to rearange the data, so they are easily put in a pivot table.
Push on the button on the output sheet and the date will update.
After that I made an graph of the data (as example, it also can be a line-graph).

Notice my main language is not English.

Probably Vikas_Gautum put me on the right track.
I changed the macro, so there are also the values of the weekly in the table.
In the pivot table, you can also change to other jobs.

That looks like a nice solution from Vikas. However, he did not add in your requirement also to match against the term "Provider". I have added that into his formula.
