Lookup value based on range and unique ID

    Hey guys,

For the life of me I can't figure this one out today. I know there's an index and match formula in there somewhere...

I need to return, by month and for each ID (table in green), a $ value from the table in red, based on the purple table. Essentially, based on A, B, or C, the value will be different based on where in the range breaks the cell value (from purple table) falls.

    Any help would be appreciated.

    Thanks!
