Data in columns B through G is retrieved from a master list by VLOOKUP function with comparison values located in column A
Dates in columns H through M auto-populate by formula based on source dates in column D.

I need to know
1.) which reports become due during the same month/year for all dates and
2.) which reports become due during the same month/year >=TODAY()

The output should look like this example for October 2014:

Thanks a Million, boopathiraja, I am very impressed with your solution. This works perfectly to identify which patients are due for their pre-determined visit date in a given month. Can Excel by any chance auto-populate only those rows (A1:G267) that return a result and not show rows with blank results? In other words, if Excel finds a date in any of columns H:M, can Excel pull the related Data residing in the same row in columns A:G from the range in the data worksheet? I am only familiar with using a filter which would involve extra steps.
Sorry about the terrible jargon. I have not passed the Excel dummy stage, as you can tell.
