V-Look Up or Index Match Problem (HELP!!!!)

Hi, I am hoping someone on here could help me figure this problem out.

I need to find an individuals amount on a spreadsheet. There are multiple people on this list and each person is on there a couple of times. What separates multiple names on the list are the "types" of amounts. Some people are listed and it shows their "1-ERN" amount, or their "3-TAX" amount or their "2-DED" amount.

I need a formula that will find Jody (ID#:89558) and give me only her "1-ERN" amounts. You will find that she as well has two 1-ERN amounts listed. One for the beginning of October and one for the last part of October.

I have tried figuring this out with the V-lookup and cannot make it work. I feel like that is the right formula, but that I may need a nested v-lookup within my orginal formula to make it work.
