can you pull data using VLOOKUP and list chronologically?

I am pulling data from "spreadsheet 1" and having it populate in "spreadsheet 4". I'm currently using the =VLOOKUP("YES!!",'Physician Response Time Log'!A4:B1000000,2,FALSE) and if the physician name from column 2 that has a "YES!!" beside it in column one populates on spreadsheet 4 in the cell where the formula was placed. I am trying to set up the workbook so that every time a new YES!! is placed in column one by a physician's name in column 2 the physicians name populates on spreadsheet 4 in the list chronologically as new data is inputted into the spreadsheet. Any suggestions on how i can achieve this?

Attach a sample workbook. Make sure there is just enough data to make it clear what is needed. Include a BEFORE sheet and an AFTER sheet in the workbook if needed to show the process you're trying to complete or automate. Make sure your desired results are demonstrated, mock them up manually if needed. Remember to desensitize the data.
