How to find a text and put the date before it?

Hi,

Been stomped on the formula to use.

Hope someone can help me on what formula should I use so that it would automatically put the Count and the Sum of the columns in Data A, B and C
for each person on the table at the bottom of the picture. The data should look like what's on the table at the bottom of the picture.

Really appreciate the help.

Hi, welcome to the forum

It would be better if you had uploaded a workbook, instead of a picture of it.

Having said that, take a look at using teh SUMIF() and COUNTIF() functions?

If you still have a problem, upload a small (clean) sample workbook (not a pic) of what you are working with, and what your expected outcome would look like.
