Consolidate Data In Multiple Rows and Columns Into One Row

Hello, and thanks for your help in advance.

I have received a download that puts data into multiple rows and columns, and I'd like to consolidate this into one row if possible with one consistent formula (I have formulas, they are just not consistent). I'd rather have a formula than a macro if that is doable.

I've been using the INDEX function along with the qualifiers ROWS, INT, COLUMNS, and MOD (COLUMNS) thanks to an example I found in this forum (sorry I don't have the link to credit the person). However, it's not quite getting me where I want to go yet.

The file I have is confidential, so I didn't want to paste the real thing. Hopefully, you can get where I'm going with this. I'm trying to get the formulas to work on the "Output" tab.
