Dynamically populate a range using criteria against another range

I have a workbook (dummy attached) that is used to input various parameters on incident tickets on one of the sheets. The first column of the "Inputs" worksheet contains a unique Incident # (or ticket # if you will). The last column of each row (labeled PHASE) is a calculation that determines what phase the incident response is in (1 through 5). Phase 1 is achieved when the "Date Reported" column has value, Phase 2 when "Date Acknowledged" has value, Phase 3 when "Tier II Engaged" has value, Phase 4 when "Tier II Acknowledged" has value, and Phase 5 when "Closed" has value.

I'm now needing to create a dynamic "dashboard" to provide management with a quick look at only the open incidents that are currently being worked. On the "Dashboard" sheet of the attached dummy workbook, I have 4 sections to represent the four Phases. I want to dynamically populate the rows of the Phase sections with an Incident # and Last Activity date when there are rows containing their respective Phase value on the "Inputs" worksheet. The source value for "Last Activity" is different for each of the PHASES. For PHASE 1: "Date Reported", PHASE 2: "Date Acknowledged", PHASE 3: "Tier II Engaged", PHASE 4: "Tier II Acknowledged". If you refer to the dummy workbook, you see that I manually entered values on the "Dashboard" to follow the logic that I'm trying to explain.

Since this is a dashboard for open items, I'm not interested in anything that is PHASE 5.

Any suggestions or recommendations are greatly appreciated!

Couldn't you make it a table that can be filtered by individual or multiple columns? See attached
