Small1, Small2, Small3 marks are same Index function and Cell address function not working

Hello sir,

I want to find small 3 subject name and cell address. I already found small1, small2, small3 but subject names and cell addresses are not finding correctly.

Small 1 and Small 2 marks are same it is working but Small1, Small2, Small3 marks are same Index function and Cell address function not working i.e. Subject 2 and subject 3 are showing same.

I already used a formula index and cell address but it is not working above said problems.

please help or give some solutions.
