Multiple criteria for selection and display with index match or vlookup?

In column F, I would like to be able to get the date when something was "worked", taking into account the criteria in column H, I, K in the "INCARCARE" worksheet. The value in column F should be obtained by comparing the criteria in column H, I, K in the "INCARCARE" worksheet, with those in H, I, K in the "EXECUTIE" worksheet.I tried with VLOOKUP, but I don't think I understood exactly the parameters.
