Getting error N/A in index-match formula

Hello friends,
I’m getting N/A error in the index-match formula.

Please suggest me a correction to get perfect require output.

Your formula needs to be this in CS3:

=INDEX(Personal!$G$9:$G$5000,MATCH(Information!$CR3,INDEX(Personal!$B$9:$B$5000,0)))

then copy down.
