Date formatting and calculating age in months.

I am having a difficult time formatting my date columns so that they are mm/dd/yyyy. I need them to be in this format but even when I try to customize the date formula it doesn't work. I also have a lot of data to enter and one of my columns is age in months. I have one column with a start date and one column with an end date (in the format mm/dd/yyyy) and I need to calculate the amount of months between them.

Please attach a sample workbook. Make sure there is enough data to demonstrate your need. Make sure your desired results are shown, mock them up manually if necessary. Remember to remove ALL confidential information first!!!

This is a sample/example of what I have to do. I need the months calculated that occurred between the DOB and the collection ate. This is going to end up being the case for a lot of data so I need a formula that will work.
