Match Index, Small Index with Concatenate

I need help with a match index formula at first. After this I am going to change the pathway by using Concatenate, so people can just copy and paste the rows and not have to manually enter a lot of information. -The formula is in the Example_2 workbook.
I was trying to use VLookup, but this is not going to work as ranges will constantly be changing and so forth.
