Array Formula Issue

Trying to get an Array Function to work by looking for two different criteria. I can get it to work with one criteria but when I do the AND it gets all screwed up somewhere and I do not know where or how. Please see the attached file. Greatly appreciate the help.

I've found that AND doesn't seem to work well with array formulas. Rewrite the conditions as nested if statements.
