Sorting data from one column into another by using a custom range

Hello all,

I work in analytics and I'm trying to sort data from one column into another through a custom set of ranges. I have about 1400 pieces of data in each column. I have the submit month for each form. It has specific dates from Jan 1 2015 up until this month. I'm trying to sort all the submit month dates into the quarter they fall into.
(Jan 1-Mar 31 would fall into Q12015
Apr 1- Jun 30 would dall into Q22015) And so on every three months is one quarter up until July of 2016.
So a specific example would be
In column B (Submit month) row 3 (B3) 12/1/2014, I would want then want it to say Q42015 in Column E.
I tried the lookup formula but was not successful. I put in
=LOOKUP('1/1/2015' to '3/31/2015',B:B,"1Q2015",'4/1/2015' to '6/30/2105',B;B,"2Q2015",'7/1/2016'to'9/30/2015',B:B,"3Q2015",'10/1/2015'to'12/31/2015',B:B,"4Q2015",'1/1/2016'to'3/31/2016',B:B,"1Q2016","4/1/2016'to'6/30/2016',B:B,"2Q2016")

Any suggestions or help would be great
