Use a particular list based on criteria

I am looking to build a registration form for a sports event.

Based on Age, Gender, and Level the lists will change.

The lists I want are

~ All
~ Junior Male Novice
~ Junior Female Novice
~ Junior Male Advanced
~ Junior Female Advanced
etc etc

So if their age is under 18 but and they are novice only that list will appear.

First I was thinking of a dependant list but it needs key words i.e. Film > Genre > Comedy etc etc.

Im thinking a nested if but do I write it? Or is there a better way?

Thanks, it was actually her site where I found the first piece of the puzzle. I need to get it based on a few criteria fields rather than just the one. I will dig in deeper. Thanks.
