    Change data structure using Formula or Power Query ( Get & Transform)

    Hi,

    I need to restructure the data in an existing table as shown in the attached file.
    Im trying to find a formula solution and if possible an alternative using Power Query also

    You can also do by formula.
    Look attach file.
    In summary sheet extract unique name from sheet1
    Accordingly revision number "submit" & "return" Date extract by formula index match.
    In sheet1 add helper column. "E" also create table in case in future add data in sheet1 auto pull down in summary sheet.
