Custom format for date with week number

Hi,

I need a formula to get B2-B18 (custom format for date) in reference to A2-A18 (date). last week of the month is for next month.

ex.
Cell A2 = 10/30/2017
It should reflect this in this format = Nov-17-Week 1

I have attached the file for reference.
