Extracting data from a table using INDEX MATCH

How can i pull required lines of data from a table based on a two critieria, i thought i coul use INDEX MATCH, but i'm not sure i can have multiple search criteria.

what im trying to do is return all rows when the "v at Fault" is selected, i can do this for the first row, but it will only pull the first row

i have attached a sample sheet to show what i have done

that looks like it will nearly work, however the original sheet had a cell selection, so when that cell had a valu in it, the lookup range was "v at Fault", on sheet1, then this would return the row, or all rows with that value, within a two week period

I've made a slight change to your formula and ity works perfectly

how would i make it so it would only search within a two week period
