Help with a formula to pull specific person info from a spreadsheet

I am attempting to finish off this spreadsheet I have been working on but I can't get it to pull the individual only info into a separate tab. I have attached my excel document. In the first tab is the information that is what everything will pull from. That data will be manually entered. The second tab pulls that information out by month. In the third tab is where I encounter the problem. I am trying to pull out one person's information at a time, this would be into the third tab. I was given the following formula to use.

    =IFERROR(INDEX(Spreadsheet!$A$3:$K$18,SMALL(IF((MONTH(Spreadsheet!$B$3:$B$18)=1)*(Spreadsheet!$B$3:$B$18<>0)*(Spreadsheet!$A$3:$AB$18="Doctor Strange"),ROW(Spreadsheet!$A$3:$K$18)-2),ROW($A1)),COLUMN(A$1)),"")

I have given an example of what it should look like in the tab called "Doctor Strange Stats by Month"

    I hope you can help.

    Thanks all!

If you're open to using a pivot table, here is an example.

Re: Help with a formula to pull specific person info from a spreadsheet

I am not opposed to using a pivot table. Though I am not very familiar with them. Is there a way to get the pivot table to take specific months as well (1-3 months chunks)?

Also, how would I change the number of names for the pivot table itself? I guess my question is, if I enter a new name into the spreadsheet how could I get that to auto-populate into the drop down menu for "gapper?" Then is there a way to get the Grand Total numbers to show up and the "Blanks" to show up as 0 or no value listed?

I guess, I had been leaning towards formula's because I was a bit more familiar with them.

    Really appreciate the help.

Hello natesamuelson and Welcome to Excel Forum.
With the original data in table form, the pivot table can update to show additional names by selecting refresh on the Pivot Table Options tab (2010 version)
If you choose one of the months and right click the mouse you will get an option to Group the data > Select Quarter and deselect Month and Day to get 3 month chunks.
To get the Grand Total to work you'll need to put the Errors/Correct in the Values field.
Since there is only one 'Reason' per Claim Number you could combine in the Notes field.
The proposals mentioned above are modeled in the attached file.
