count number of records per month based on criteria

Hi

I want to count the number of times someone has called per month based on their ID number.
On sheet 1 I have the data and on sheet 2 I have the summary report.

Attached is sheet 1 with the dataset and sheet 2 with the summary report and then an example of the outcome that I want on sheet 2

I have tried using a pivot table but it doesn't display as I want.
The data sheet (Sheet 1) will be populated monthly and I can change the headings for the summary page for the months.

Hopefully that makes sense.

I can see no reason why a Pivot Table can't produce the information you want.

Please upload the workbook so that we can see your data in context and check.

I think I have just uploaded the workbook. I'm hgaving issues with the paperclip attaching, so hopefully this has worked.

That would work thanks, but this would mean that the data for each type is not next to each other on the report. Is that possible?

Thanks for the newer version but I don't know if a pivot is the best option as it doesn't show the blank months
