Help, confused with index match return cell adjacent to a value

Hello Everyone,

I really hope someone can help me with this as I have confused myself completely.

It would not let me attach a file, so hopefully the screenshot attached okay (it looks okay in the preview)

I have tried playing around with index match and confused myself completely trying to get my head around array formula, I was trying to stay away from using the headers as there are more than one called 'DOE'. I think I would need to index match the Registration to find the row and then find then match the date on that row then somehow return the cell next to it. I am sure it is simple for excel experts but I am a little out of my depth here. Any help would be really appreciated.
