    VLOOKUP formula that matches a date and returns sequential text from a separate worksheet

    I am trying to streamline a data entry process for tracking central line associated blood stream infections (CLA-BSI). I have attached an example of what the data might look like in the "TestData" worksheet. This data would be appended for each month of data. In the "Forms" worksheet the nurse would enter the first day of the month in the following format in cell J1 (11/1/2018) which will return the spelled out month and year. Using this date from cell J1, the formula I need would look for this same month/year combination in TestData worksheet, then input the data from TestData column A if the dates match. For example:

    In Forms worksheet cell J10 I would like to write a formula to VLOOKUP(J1,TestData!H2:H9) if any of the dates in the TestData H column match cell J1 from Forms, then return the first value in Column A from the TestData (i.e. Patient 1). I would like a way to enter the next patient name in the list as long as the dates match for the next patient name section in cell J12 and so on. So a sequential listing of the values from TestData column A as long as dates match.

    Hopefully that makes sense, but should be clearer once you view the attached example file.

    How about this in J10 (it's an array formula and so needs to be confirmed with Ctrl+Shift+Enter) then copied and pasted into J12, J14, J16 etc.
