Extracting Multiple Matches with Multiple Criteria

Hi everyone!

I've got a little array formula that I'm using to return some values from a table. It looks like this:

That works quite nicely to return all the entries under the Players column where they match Region in cell G1

But now I'd like to start adding more criteria. I've got another column labeled Score and I'd like to add a criteria so that I'm only indexing those Players with a score of 5 or more.

I can't figure out where to add this in! Any help would be much appreciated!
