Need Index match formula based on two conditions

Hi,
See attached example.
I would like Excel to lookup a column of values based on two conditions (date and scheme) and return a sum based on the month.
I've done a bit of reading on this. Seems SUMIF can be used with Index and match - other posts suggest SUMPRODUCT.
Can someone help with the formula?
