using a pulldown as a match to pull data from another sheet.

HI,
So I am having a problem with a function I am trying to do.
Ok this is a workbook with 4 sheets.

on the first sheet there is a pulldown in cell 12B that lists facilities by the town they are in

I want to pull data from a different sheet based on what is selected in that pulldown and populate 16B with the address

The other sheet looks like this:

as you can see 2 CDE make up the address. the Property column in the actual version matches the property name from the pulldown.
I want to pull those 3 columns into text to populate the Street column (16B) on the first sheet.

I can match and get a row #, I can index and get some weird results. But I cant get it to populate with the contents of those 3 cells as an address in the 16B cell

Can anyone help me with this? Seems as if it should be easy, but I just cant get it.

I put dummy information up in the second form to avoid violating HiPAA, just assume that prop1, prop2, etc are the property names.
My bad on the syntax conventions. I don't do much outside of *nix unfortunately, bit of a cave in that sense. But it is noted and will be correct from now on.

So if Johnstown said prop1 or vice versa, what would i do?
keep in mind, thats a full drop down menu with many locations. but if you tell me the syntax of the functions or help me with it, I can put the specific data later.

nm I got it

sure, but it's elementary.
I made Cells on the main form for each of the individual aspects of the address.
Then I just did an INDEX MATCH function to match for each individual Cell, instead of trying to concatenate them.
