Problem looking up text data from 2 columns to return a correct value

Hi everyone,
I am trying to look up data from 2 different columns that are arranged differently to come out with the correct values.

1) I thought that VLOOKUP with an "&" will do the trick but it was not working for me.
2) Also, how do I return mulitple values based on my example? Please see Excel worksheet attached.
3) If one of the 2 lookup columns has multiple words that don't match exactly but do match for the first 2 to 3 words, is there a lookup method for that scenario?

Please attach a sample workbook (not a picture or pasted copy). Make sure there is just enough data to demonstrate your need. Include a BEFORE sheet and an AFTER sheet in the workbook if needed to show the process you're trying to complete or automate. Make sure your desired results are shown, mock them up manually if necessary.

Remember to desensitize the data.

Click on GO ADVANCED and then scroll down to Manage Attachments to open the upload window.
