INDEX with second criteria

I have the following formula that I'd like to expand on:

Currently it matches value in S1, I'd like to also match value in S2.

This now works with the following:

My next goal is to add two more conditions with different ranges:

Please Login or Register  to view this content.

This formula however is not working?
S4 has value of *p (WILDCARD p)
S5 has value of *F (WILDCARD F)

Is this possible?
