match index and if for file link

HI EVERYONE.
i have file in which i have scheduled the cleaning tasks of machine and it's parts.
there are two type of cleaning first on regular basis and second on particular occasion.
which i am going to handle with google forms. for daily cleaning schedule there is sheet "form responses 5" which belongs to sheet 1! row no. 12-13.

and row no. 14-15-16 belongs to form responses 1. data of row no. 14-15-16 will be occasionally.
everything is fine. only what i want is that this sheet should be automatic.

in row 11 every column which contains "P" has a formula. (please check the formula, if i will elaborate here, it will become a mess.) so my requirement is that in row 11 where ever is the column of "work". it should be auto fill with "normal", "cotton ", chenille".

and in row 11 where ever column of "before" and "after". their regarding link should also filled by itself there.

for more result please see sheet desire result. any changes will also be acceptable but want to have this as solved
