find the date range indicated by the first day of the week in a chart? match-index?

I have a range of dates that, depending on two different service levels, need to return an amount in a chart.
Date is a column, the two service levels are a column, return amount is in a chart.

So - if the date is 12/23/2019 and the service level is ground or air, I need it to go the chart, find the date range indicated by the first day of the week, match that to ground or air in a row across the top. I can't use Vlookup & match because the data isn't in order.

What I would really like this thing to do is understand the correlation between mode and service and do that automatically, then be able to look up the service FSC cost.
