Index match with 3 criteria not working

Hi all;
I have a 3 criteria, Date, Project name and Plan/Achd. quantity for the day. i have given a formula like this
=INDEX($C$3:$N$12,MATCH($C18,$A$3:$A$12,0),MATCH($B$18,$C$1:$N$1,0),MATCH($B$4,$B$3:$B$12,0))

it is working for only plan column , where as for achieved it is not working. please help

can u just tellme what did you do.. what is the use of adding +1
