Pulling Text from multiple cells with 1 cell being a date

Hey guys, I'm having an issue as to where I can pull text from multiple cells, but if the source cell is a date, the value it pulls into my destination cell, is returned in numbers. I've tried to format the Destination Cell as date, but it still returns the value in its number format. how do I fix this?

This is an example of basic string formula
