Lightbulb Hourly Rate Calculator in Excel for Film Budgets

how would one create a calculator like this in excel?

    filmmakersproductionbible.com/hourly-rate-calculator

I want to implement this into my budget docs for production to make sure rates are fairly addressed. With that figured out, I could build it into the formulas in my spreadsheets.

Thank you all for you time and effort!
