Requesting tutoring help with Nesting INDEX in Concat

All,

I'm signed up for an on line Excel college class and I am totally stuck on how to nest Index in Concat. I can either get the results to show up from the Index or the Concat but not both. As I've been stuck on this one point since last Tuesday I am seeing tutoring or homework help, not a direct answer for me to copy and paste.

I've reviewed our on line text book as well as searched the Internet, as well as contacted the instructor (no help really). I certainly do not want anyone to simply send me the answer, I just am hoping someone can direct me as to how I can nest index in concatenate.
