HELP with fixing a formula on the attached spreadsheet

    Hi everyone,

On the attached spreadsheet, I am trying to create a formula in columns F, G & H that will show 'YES' if the consumer was admitted to the BHU or GPU service line within 30 days, 60 days & 90 days after their discharge date/time from the CSU service line in column L under the table to the right of the gray divider titled CSU Patient Encounters. The formula that is currently in columns F, G & H does not seem to be giving me the correct results. When I tried to validate the results by searching the consumer ID #s in the spreadsheet, I am seeing some patients are being omitted and a 'YES' is not appearing in the row even though the date of their admission to BHU or GPU service line was within 30, 60 or 90 days from the discharge date from the CSU service line in column L on the table to the right of the gray divider. Please let me know what I am doing wrong or provide me with the correct formula. Thank you!

Which specific rows/examples to you see being omitted and "yes" not appearing when you think they should be?

**Array formulas are not entered in the same way as 'standard' formulas. Instead of pressing just ENTER, you first hold down CTRL and SHIFT, and only then press ENTER. If you've done it correctly, you'll notice Excel puts curly brackets {} around the formula (though do not attempt to manually insert these yourself).
