Return values in the first and subsequent non blank cells

I am looking for functions which will return the values in the first and subsequent non blank cells.

The attached workbook is about recording observations of babies who are born prematurely. The range B18:D22 require functions which find eg the first observation date and the data relating to that observation date, then the next observation date and the data relating to that observation date etc. The actual observation dates recorded in row 9 need to be the starting point for the functions in the range B18:D22. The process the user expects to follow is: enter the actual observation date then choose observation code.

The actual workbook contains far more data collection and helper cells than this "public" cut down version.
